Eisenhower Primary Care 365 offers regular, ongoing communication between patients and their providers, supporting the development of trusting, lasting relationships that contribute to positive outcomes and better quality of life. A part of the Center’s relationship-based care philosophy, participating patients can connect with their care team 365 days a year through extended face-to-face onsite visits and by telephone during the clinic’s Monday-Friday hours. Night or day, patients also are encouraged to communicate with their care team members online through our Web services partner, RelayHealth. Routine follow-up with patients ensures the Primary Care 365 program produces and maintains high levels of patient satisfaction.
