A personalized primary care model, the Eisenhower Primary Care offers regular, ongoing communication between patients and their providers. The program makes care available when and where patients need it, and it supports the development of trusting, lasting relationships that contribute to positive outcomes and better quality of life. A part of the Center’s relationship-based care philosophy, participating patients can connect with their care team through extended face-to-face onsite visits and by telephone during the clinic’s Monday-Friday hours. Night or day, patients also are encouraged to communicate with their care team members online through our Web services partner, RelayHealth. Routine follow-up with patients ensures the Primary Care program produces and maintains high levels of patient satisfaction.
